Officials cite income misrepresentation, ineligible applicants, and unauthorized “phantom enrollments” by insurance brokers as key issues. The administration has restored stricter verification requirements and increased oversight to protect taxpayer funds. The findings are expected to renew debate over ACA enrollment policies and program integrity.
