Pilot’s Deadly Missteps Led To Crash That Killed 67 In Army Helicopter Tragedy, Report Finds

The recent revelations surrounding the tragic Black Hawk helicopter crash that occurred on January 29 have shed new light on the series of errors that led to the mid-air collision with an American Airlines jet, killing 67 people. A key factor now emerging is pilot error by Captain Rebecca M. Lobach, 28, who allegedly failed to follow critical instructions, including a suggested course correction moments before the crash.

According to The New York Times, Lobach ignored guidance from her co-pilot, Chief Warrant Officer 2 Andrew Loyd Eaves, who advised her to turn left to avoid the oncoming jet. Transcripts show air traffic control had authorized “visual separation,” putting the responsibility on the helicopter crew to maintain safe distance. Lobach confirmed the clearance but failed to act decisively.

Despite having no known medical issues and a decorated service history, questions remain as to why she did not respond. Investigators also noted communication dropouts between the helicopter and control tower. A final NTSB report is expected early next year.
